Revisiting the Fasciocutaneous Perforator Cross-Leg Flap

چکیده
DESCRIPTION A 23-year-old man sustained an open pilon fracture of the ankle. Fractures were plated. A week later the medial wound dehisced with plate exposure. Good local tissue coverage was not available and free tissue transfer reconstruction was contraindicated because of lack of suitable recipient vessels, as demonstrated by computed tomographic angiography.

Sural Versus Perforator Flaps for Distal Medial Leg Wounds.
Soft tissue coverage of distal medial ankle wounds is a challenging problem in orthopedic surgery because of the limited local tissues and prominent instrumentation. Traditionally, these wounds required free tissue transfer to achieve suitable coverage and subsequent bony union. To better respect the reconstructive ladder and to avoid the inherent difficulty of free flap coverage, rotational fl...
متن کاملPropeller Perforator Flaps in Distal Lower Leg: Evolution and Clinical Applications
Simple or complex defects in the lower leg, and especially in its distal third, continue to be a challenging task for reconstructive surgeons. A variety of flaps were used in the attempt to achieve excellence in form and function. After a long evolution of the reconstructive methods, including random pattern flaps, axial pattern flaps, musculocutaneous flaps and fasciocutaneous flaps, the reapp...
متن کاملPerforator-sparing transposition flaps for lower limb defects: anatomic study and clinical application.
The local fasciocutaneous flap has the advantage of low donor-site morbidity when used for the coverage of lower limb defects. However, flap reliability remains a major problem with its use. The purpose of this study was to determine the feasibility of preserving perforators to the tip of conventional local fasciocutaneous flaps to improve its vascularity. The technical considerations of raisin...

Further, a sufficient understanding of the wound characteristics, donor and recipient vessels, and design for insetting should precede the surgery. Lastly, the skin graft over the muscle flap is less durable than the fasciocutaneous flap and tends to form a callus. To prevent this complication, a p...
متن کاملPerforator Flaps for Reconstruction of Lower Limb Defects
BACKGROUND Reconstruction of soft tissue defects in the lower third of the leg remains challenging. Anatomical constraints limit the local options available for complex defects especially lower third of leg. Local flaps based on perforator vessels are raising interest in reconstructive surgery of the limbs. We present our experience with perforator flaps for reconstruction of soft tissue def...
